Pancreatic abscess at 14-years old boy

We report a 14-year old boy with pancreatic abscess that is very rare in infancy and childhood. He was admitted to hospital because of abdominal pain with variable intensity and character, accompanied by fever, nausea and vomiting two weeks before. He also lost 5 kg of body weight. Abdominal ultrasonography revealed a pancreatic area abscess, and guided percutaneous needle aspiration was performed. Cytological examination of material (pus) obtained by this procedure confirmed pancreatic abscess. Wer could not define its aetiology by the available diagnostic procedures. The abscess persisted for about 70 days after the first symptoms of illness and one month after treatment and then regressed spontaneously. Treatment included one month therapy with cephalosporins of third generation, metronidazole, fluconazole and local instillation of gentamicin. In spite of the fact that there was no sign of regression of the abscess our patient was without symptoms with normal ESR and negative CRP. These are features of a self-limited diseases such as cat-scratch disease, which our patient probably had because was often scratched by his own cat.
